What to do if my health insurer has denied benefits for my son?

Question Details: My son had a lot of dental work to be done but due to his anxiety the dentist recommended that he have anesthesia for the procedure. The insurance company paid for the dental work but has denied the anesthesia bill. Originally the doctor estimated 2 hours but it turned out to be 6 hours for $5,000. My insurer denied 2 appeals for several reasons but contradicted themselves. I filed an application with the state department of banking and insurance. Now I've gotten a letter with a different review and different reason. This has been going on for over a year. it's wasting my time and energy and is emotionally upsetting.
